ATM / Access Visa Debit Card PIN

Use the following form when an ATM / Access Visa Debit card has been damaged, or when a PIN has been forgotten. In either situation, a new card and new PIN will be generated.

Upon submitting a replacement card/PIN request, your current card will be deactivated within three business days. If this will cause hardship, cancel this request and call the appropriate phone number below.

DO NOT USE this form to report a lost or stolen ATM or Access Visa Debit Card! Doing so will not protect your rights. For lost or stolen ATM cards, you must call 260.471.8336 or 800.728-8943 between 8:00 a.m. and 6:00 p.m. EST, Monday through Friday, 9:00 am - noon on Saturday or 866.304.8684 after normal business hours.
